I did the 10 miles version of this event. The starting area was huge, there were a lot of people entered, I think it might even have the second largest number of participants of a race in Belgium after the Brussels 20km. Bag drop, number collection and toilets were well organised.The start was organised into 3 waves, each being half an hour apart. I was put in the last wave. While this meant for easier organisation it does dilute the large event atmosphere somewhat and some of the crowds had gone by the time slower people in wave 3 went around. The course was good and takes you through the more interesting parts of Antwerp. Aid stations were sufficient. If you are travelling by train to Antwerp for the race allow plenty of time to get there and get home because the start is on the other side of the river. You have to walk through the tunnel or get a free boat but either way it takes some time.There were medals but I feel they could do with a more exciting design. Overall I would do it again but perhaps not every year.
